Nurses and Midwives are required to notify the Board of change of address in writing within three months. You can access the form below.
CHANGE OF NAME
It is necessary for you to provide legal documentation supporting your request for a change of name (eg marriage certificate). You will need to return your current certificate of registration or enrolment before a replacement can be issued with the name change.
